For new clients, the first step is to call us or fill out this form, and we’ll get back to you shortly to schedule a FREE 15-minute consultation. This session allows us to discuss your needs and determine if our practice is the right fit for you. If it’s not, we’ll happily provide referrals and answer any additional questions you may have. After that, we’ll share our calendar so you can conveniently book your appointment at a time that works for you. If needed, we can assist with scheduling.

I do not charge for calls under 10 minutes. For calls longer than 10 minutes, a pro-rated fee will apply. As for paperwork, I prefer it be addressed during your appointment so we can complete it together.
I ask for 24 hours’ notice if you need to cancel or reschedule an appointment. If you cancel with less than 24 hours’ notice, you will be billed for the missed appointment. Cancellation or rescheduling is easy through our booking system, and you can also reach out by phone or email for assistance.

I primarily work online but also offer in-person appointments at various locations across the Chicago area and in Dyer, Indiana. Currently, I see patients in-person in Burr Ridge, Oak Brook, IL and Dyer, IN, and am continuing to expand to additional locations.
